-One (01) Form to be filled and sign by the applicant at the Embassy, or a mailed from (to be requested) notarized (the seal of the public notary MUST be on the form and not attached).
-Notarized proof of identity (valid passport or National Identity Card). All other documents are not considered valid.
-A non-refundable US Post Office money-order only acceptable (individual) made payable to the order of the Embassy of Algeria in the amount of US $1.00.
-A self addressed prepaid envelop.
